My 2 male cats do this. They will shoot of out the room where the litterbox is and down the stairs and act wild as a march hare!!!!! One of them also meows afterwards, sometimes too. I look at it as a warning to hold my nose LOL I have never thought of it as an indicator of something being wrong with them.
