I highly recommend cleaning where he's peed with Nature's Miracle. You can get it a most pet stores. Even if you've already cleaned it with another product I'd suggest going back over it with Nature's Miracle.
I also would not let him in your bed for awhile. Get him his own bed and put it on the floor next to your bed.

Maybe a crate/carrier at night for a while until she learns not to sleep on the bed.
