Well...this is what I have to say about her going to the bathroom. If you WITNESSED her going in the house, I would make a sharp noise, like banging your hand against the wall. Hopefully, she would look at you and then you could take her outside. If you said "NO!" then she might learn that you don't like to see her go.

Also, if you find poo in the house, don't punish her because by the time you find it, she'll have completely forgotten about it and won't even know why you are yelling at her.

Praise her alot when she goes outside.

I know, sometimes it is REALLY frustrating!
