Are you referring to the optional blood work before the spay/neuter operation?
If yes, then I personally would always get the blood work before the surgery. It tests the kidney and liver functions before giving the anesthesia. If they aren't working well, then anesthesia may be more of a risk.
Also, because there are many drug options available, the blood work will also help determine the best one to use for that specific animal.
