Don't smack her for any reason.
She thinks you're playing when you smack her for biting. Instead try holding her jaw closed, say "No bite" and let go. Then walk away from her for a while.
Since she goes to the door when she wants out, try hanging a bell where she can reach it next to the door. When you take her out, ring the bell. Maybe even use her paw to do it. Eventually she will ring the bell to go out. Be sure to take her out when she does.
You might want to set up a schedule to take her out, like every two hours. Set a timer to remind yourself and stop whatever you're doing. How does she do overnight?
Don't make an issue of it when you find a puddle. Clean it up with vinegar and water so she won't want to use the same spot again. Never rub her nose in it. She has no idea what you are trying to tell her by doing this.
