I believe in what the Bible teaches on the subject:

"Do not be deceived, God is not mocked; for whatever a
man sows, this he will also reap. For the one
who sows to his own flesh will from the flesh
reap corruption, but the one who sows to the
Spirit will from the Spirit reap eternal life.” Galatians 6:7-8

To me this means being good and honest even when people aren't watching. I have done some mean things and I know I personally felt more guilty when I did things that no one found out about as opposed to when I got caught. Getting caught meant part of the punishment was over! When no one but you knows of a wrong deed, the pressure of waiting for the axe to fall is overwhelming. I really think that people fall ill due to this kind of guilt.
I don't know why bad things happen to good people. I like to believe that even suffering has a purpose, even if we as human beings can't understand what that is.

The book of Job comes to mind on that.

In honor of John Lennon..."Instant Karma's Gonna Get You!"
Although I don't think it's always instant. Anyway, works for me.
