This is so sad to hear. But at least whatever time he may have, is now being spent with lots of love, both human and cat companions. You will know in your heart when its time to let him go. If you dont think that time has come, then keep loving him until it does.

This is the most heartbreaking story I have ever read. I can't even imagine how you must feel. But if Alex isn't himself, as you say, then the time is drawing very near when you must do the last act of kindness for him. He has felt and is feeling your love and warmth and the companionship of this fur buddies. But it's heart wrenching to think that he's scared and wondering why he feels the way he does. I wish I could do something to help you. I have been sending healing energy and lots of prayers for you both.
