If she does have live puppies you won't want to have her spayed at the same time as the c-section unless you plan to bottle or tube feed.

If you have her spayed, the odds will be great that she won't be able to provide milk for the puppies and may even reject them.

You're right. She has a 50-50 change of accepting them and being able to provide milk for them if she is spayed.
