PLEASE, go to your local medical society/welfare department/somewhere that can give you some help! This sounds like more than you can handle on your own. You need someone to help you. And there is no shame in asking for help.

I wish I was closer so that I could try to help. We have a local League for the Blind and Disabled that helps me with interprators when I need one for a nonhearing elderly person. Maybe you have something like that and they could help you with a case manager.
