Adoption contracts

A woman I work with is starting to screen people to adopt her huskies puppies (she is also going to try to get some updated pictures for me this weekend...they are already 4 weeks old!)

Anyway, she would like to put together an adoption contract, saying the dogs should be altered, and that they would be returned to her if they couldn't be kept for any reason, etc. Would anyone be willing to share the adoption contract they (or their shelter uses) as a starting point?
