~PT Pen Pals~ Questionnaire

This is for everyone who is participating in the PT Pen Pal's for November and December. Please fill out the questionare below and email it to [email protected]. I will NOT except any by PM, because it will be too confusing for me. The deadline is October 20th, please have them in by that time! After i recieve all of the questionare's which i hope will be by the 22nd () I will begin to sort them out and give you your new pen pal.
*Reminder, you don't have to stop mailing your old Pen pal. This is a chance for you to get to know other PTers that you may not know too well. Hopefully you've already gotten to know your old Pen Pal.
