So badly that you will not do something you enjoyed or never thought twice about???
I scared my self out of rock climbing.
I made it up a chimney*about 30 feet on my own and panicked. I walked around the corner from where my friends were and thought I'd climb in with no safeties in.....no rope or harness.
For some reason my mortality hit me in the face I couldn't move and had a panic attack. I finally MADE myself move. I still get an ugly feeling in my stomach thinking about it.....even while watching anything that has to do with heights on the telly.
*a chimney is two walls that are close enough to climb by
putting your back on one face and your feet on the other.
You scooch your back up, move your feet, scooch up again.

I talk myself out of roller coasters all the time... but then I"m the same one who will leap at a chance to go rapelling- the higher the drop- the better- and I insist on doing it head first!

I've scared myself at work a few times. If a telephone pole has sharp objects below I start thinking about how I'm going to fall and it's going to impale me. I wind up getting myself all worked up. When that happens, my fingers are always so sore the next day. I think I subconsciously tense up and hold on like a cat.

One night after getting off work at 11pm, I was unlocking my car door, saw my reflection in the window, and thought someone was inside. Scared me to death! I had a wave of terror rush over me for just a few seconds.

I was walking around the cliffs near Seal Rocks in San Francisco and you had to schutch up on a narrow path next to the cliff to get around a curve to the more or less open spot on the other side. I froze coming back. I needed to be helped and have been terrified of heights ever since. I had been there several times before with no problem. I can look over the Grand Canyon if there is a railing but no railing? Fuhget about it.

I scare myself ALL the time...
I'm scared of the dark, heights, people following me, and so on.
Every time after I turn the TV off, and then turn the light off in the living room, I always RUN to my room because I'm afraid of someone hiding in the dark and snatching me LOL.
