Need help with writing letter of recommendation

I'm browsing online but I can't find what I really need. Would anyone with experience please tell me how to write a letter of recommendation? I've never done it before and I need to write one for the lady who works for my family helping care for my Father. She needs it for an interview with a home health agency where she hopes to work evenings. She will still be working for us so it's not a former employee type thing.

Any help is appreciated.

Her kindness (pleasant personality)
How great the care is she gives your dad (her abilites and knowledge).
Dependability as a worker (always on time to work)
Trustworthy

Craftlady - thank you for that guideline. I called my sister who has a some experience in this and she is going to write it up and email it to me. I'll have to fill in some of the specifics since I'm the one here working with her. I sent your guide to her to help.
