My 15 year old cat meow's deep in his throat a lot. Does this mean he is in pain?

Is this a sudden change? If so, you should probably have him checked out by a vet. If his behavior has changed too do not delay a vet visit. It could be a pain cry or a sore throat.
Cats all have different voices and some are just naturally low. Ambrose does a "deep throat" call when he wants attention and everyone else is asleep. Belladonna does one right before she brings up a hairball....
I have noticed that with my senior cats, those that lost their hearing developed loud and odd sounding meows... Sort of like old people who shout because they can't hear. One of mine became an almost constant talker in a very raucous voice. The vet said it was because she could no longer hear herself and "said" all her thoughts aloud.
Good luck.
