Need help with word

I'm doing a reseach report, and I have bunch of requirements for the paper. (certain text, text size, headers, spacing, etc.) I need a header that says "(My last name) 1" and the 1 is for page 1, then it'd be 2, etc. But when i go to add header there is only ones that they have their... how do i get my own? Thanks so much for anyone who can help.

Which Word are you using?

If you're using Word 2007, Go to the Insert Tab and click 'Header'. Choose the first option, blank. Click the 'page number' option, then just type in your last name before that. Then right align it.

Not sure how it is on the old word now, though

Oh well i actually have word 2000 x_x

I have your version.

You are in your document.
Click on VIEW
then on HEADER AND FOOTER

Now a small dotted box appears at the top of the document, and a header / footer menu also appears. My cursor is flashing IN THAT SMALL DOTTED BOX.

Just type what you want in that box!

On the menu bar that pops up, I have never used the "Insert Auto Text"

The next item on that menu is the icon to insert the page numbers

Next icon looks like a double plus sign. That is total number of pages. Use it if you want to put: Page 1 of 16, that icon does the 16 for you.

Next icon is to format the page number and so on.

I have Word set to show labels when I put the cursor over an icon, so I can figure out which one I want.
