Here is the e-mail I recieved today from cafepress.com.
Hopefully it answers some of your questions. :)

Hello Kay,

Thank you for contacting Cafepress.com!

We do accept International orders. The shipping rates for
International locations are $7 for the first item and $4 for each
additional item. The funds must be submitted by credit card in US
dollars to cover the price of the merchandise as well as any shipping
fees. The items are shipped via Mail Innovations and are left at your
local customs facility. From there, the merchandise is delivered
using your standard postal service. Please note that the delivery
time for International orders may vary and can take up to 10 business
days for delivery. Therefore, customers must allow ample time for the
merchandise to arrive.

I believe Canada is the same as the US rates. You'll have to contact them about that, however, on their website they have everythings you need to know when you go to order.
